If you are running 3 piece cranks have you got the small "tophat "spacer in the inside of the chainring or is the ring sloppy on the spindle ?

Might sound strange, but have you checked:

1) that your chainring isn't an oval one instead of a round one? Some where made oval.... can't remember the reason?

2) that your chain ring is centered on the spider, if its out a little on the spider is will be quite a bit out at the chain.

I have never had a biker where it is the same all the way round always seem to have a tight spot, think its normally due to the chainring and freewheel slightly out of round.

the rear sprocket is not in line with the front chain wheel. run a straight edge on the same line as the chain and you will see what i mean.
