Handle bars keep moving

WEll i've tightened the bolt up as much as i can, but as soon as i try any tricks, the bars move.

Do you think

a) i need to loose some weight

or

b) a new stem bolt wedge will fix it

when you say the bars move,do you mean the bars move in the stem or the stem moves in the steerer tube?

give stem bolt the tightening of it's life

if it strips then get a new one

It's not a Tuf-Neck style stem is it?
They have a stem bolt, but also a bigger hexagonal bolt below which needs to be really tight or the stem can slip round.

If you mean the bars move backwards and forward then the clamp's too big or the bars are too small (bars possibly crushed, or clamp-bolts stripped?).

I had this problem with a set of Haro Vectors bitd, to get around it I increased the height of the stem in the forks by about a half an inch as I found that the pressure of the stem bolt/wedge had caused the forks to either expand slightly or they wore into the forks at that point just enough to allow the stem to twist even though it was as tight as it was ever going to be.  Try lowering or raising your stem a touch and see if that helps, raising it and using a flex fighter might be the way to go.
